To print an Application after you have opened it in Adobe Acrobat Reader, select "Print" (from under "File") on the Adobe tool bar (selecting "Print" from your browser window may not work).
After printing the documents, be sure to carefully read all instructions and materials included with them. Complete the application and mail it to:
