I venture to say that the "who" may be the most challenging for the simple reason that people have potentially more power to alter perceptions of the world and self-concept than the physical space of a classroom (where), or reading out loud (what) in class (when). Your question would be a good one to ask a client or student when reviewing or looking back at progress.
