Hi - I use speech hierarchies quite often with my stuttering clients. It sounded like from reading your article that you focus on the feelings about the difficult situation (before, during and after) and not on the motor aspect. When I go to a hierarchy, it's usually for tool transfer purposes as well as desensitization. Do you have the client go into the situation with the goal of using a strategy (such as pull-outs for example) you've worked on or just the goal of speaking (not really caring at that point what comes out)?
