Hi Maria- Thaks for your question. The hierarchy may not be for everyone for any number of reasons. Perhaps the person is not ready to face fears or admit there are fears to face. In other cases, people may feel a hierarchy is too rigid and they have "failed" if they do not complete it. Or some people just don't feel a hierarchy is necesarry and find other ways to face fears and avoidances. There is more than one way out of the forest, and using a hierarchy is just one of many paths. So we introduce the idea of a hierachy to most every adult and adolescent client we see. As a team, the PWS and the SLP decide how and when to design and execute the hierarchy. I would estimate about 50 to 75% of the time the hierachy gets used.
