I would absolutely agree that rule-learning would be a much better treatment strategy than monitoring. I especially like your idea about developing pragmatic goals aimed at requesting. I used to work with a five-year-old boy with ASD who seemed to respond well when positively reinforced for appropriate requests and learned through extinction what forms of requests were inappropriate. I can definitely see that approach being used to treat disfluencies interpreted as conversational turn placeholders.
