Hello Rhian, I am not sure what you mean when you say your client wants to start at the top rather than the bottom? Generally I think it is important to start with the simplest thing possible and then add strategies. Thus (assuming your client is not a child) I would start with possibly a focus on how Starkweather describes the elements of fluency - rate, continuity and effort. Thus, if someone changes their rate, they may sound different. If they can get fluent by concentrating on flow though, they may not sound unnatural at all. However, I think that for some people who stutter very severely, they may need to forsake naturalness for fluency - especially in the short-term.
