Hello Tim, you raise several important issues. I think we often spend too much time in the clinic and not enough time outside the clinic WITH our clients helping them transfer their fluency. Hence, they remain anxious as they rarely experinece fluency in their real-life situations. They are some exciting web-based CBT programs coming out and some research is about to start looking at that.
