I'll usually start with familiar music-making just to get to know the person and form a relationship where it feels non-judgmental and supportive. I usually start using instrument-playing, since nearly everyone is more comfortable with instruments than with voice -- voice feels very personal and exposed to just about everyone, and instrument playing gives a layer of protection and removal from the self. So we might do some familiar music, then some improvisation with instruments, then add in some voice. Some people are comfortable singing right away. It depends on the person.
