Ken, in general a braille display or a speech output device connects to a usb or serial port on the computer and material is sent to it much as it is by the screen. A screen access program is used to help with organizing the screen in such a way that it speaks reasonably well and also allows for reviewing the screen. In many computers the sound card is used to provide the speech synthesizer. Of course, I have been talking about Windows but the process is similar in DOS and other operating systems.
