Maria, let me take your qiestions in reverse order. There's no way that I could actually know but so far as I know not all people stutter in their heads -- are covert stutterers. I believe that, for me, my internal stutter comes somewhere between language formulation and the motor act. Either that or it comes during the end of the formulation process.
