Those are great questions. Do you feel as though this is one of the larger obstacles in reading aloud? I think those can be obstacles either based on teacher expectations of performance or on how the child perceives he/she needs to read in the classroom. I think children as they are learning to read in general can be sensitive to their performance, especially if they are having difficulty with sounding words out or stuttering significantly. If a child avoids or substitutes words in order to cover up stuttering during conversation, anxiety might be increased during reading because you can't really avoid or substitute words when you read. Sometimes just the anticipation to read creates increased anxiety and makes the speech process more difficult. How do you feel the Demands and Capacities model may affect someone who stutters during their attempt to read aloud? I think the examples above speak to the multidimensional nature of stuttering and how demands in the environment may interact with demands the child places on himself internally.
