Hi..your comments were very perceptive! Yes, a hierarchy is a wonderful activity and important for students to assess various speaking situations...it is also good to understand that the pressures of various speaking situations. As to your second question, a student must be ready for treatment to work and I have had some students who were not ready. The good news is that many of them return when ready, especially if they perceive treatment to be positive. I also work with adults. I also hear that early treatment, while valuable, was just the beginning. I also prepare my students to understand that as they change, their fluency might also change...always keeping the door open for more treatment.
