This problem is not intractible in research. You can get the notion of anonymous calls approved BEFORE initiating a trial if it is required to validate outcomes. I don't know if your story is true or apocryphal, but there's no reason it would have to be a standing problem. I also think, Tom, that you are mixing research study results with every day life and client-clinician relationships. If an individual has a good relationship with a therapist, and sees differences in within- and outside-of-therapy performance, or begins to "backslide" from prior therapy gains, they can and should return to discuss next steps. That's what I meant when I said the client knows what is going on. If there was some good benefit from therapy that is not being maintained, I would suggest the person return to pursue the problem, rather than dropping everything and moving on to try something else.
