Regarding your questions -"I was wondering when clients who avoid come to speech therapy, how long does it typically take for them to break down these avoidences so they can receive help? Does it have a lot to do with the length of time a person has been avoiding words?" - I really don't know if one can predict a timeline, or if it is even necessary for a person to "break down" all avoidences "so they can receive help." Some avoidence behaviors interfere a lot with daily life and with communication however, and these may be avoidence behaviors that a client will need to deal with. And some people who stutter have avoided certain things for a long time - it may take years, if ever, for a person to stop these avoidance behaviors. But that doesn't mean that there aren't other things that can be beneficial in therapy, or in participating in support groups, for a person who stutters.
