I am curious which attribute- the disfluency or the articulation errors- are the biggest cause for listener error ?? I might ask him how he feels when someone needs repetition.. What does he think they think about him? He may have a cognition such as "they think I am dumb." That needs reframing to reduce his sensitivity to feed back from listeners. He needs to see the reality that others do not understand AND they care enough to want to hear it. good question.
