You can get it via orders@multilingual-matters.co.uk If I were you I'd ask the editor of one of the Brazilian journals in the area (e.g. www.clinics.org.br) if you could get a review copy. Publishers like to get their books reviewed and it'll make it cheaper for you! Peter
