Treating Covert Stutterers

I'm a graduate student currently enrolled in a stuttering class, so I'm just beginning to learn about stuttering. If covert stutters already know how to hide their dysfluencies, then what would an SLP do to treat them? If there way works for them, why would we change it? If a covert stutter is feeling anxiety and depression, shouldn't they seek a psychologist for help? My little brother is a covert stutterer and he talks in a rapid and monotone voice in order to not stutter. After so many years of talking in this way, I feel like it has become natural to him. I barely ever notice dysfluencies in his speech in person, but he is difficult to understand over the phone...He's never received speech therapy, and he isn't interested.
