Covert Stuttering

As I have learned the basics of covert stuttering in my Fluency class, what I am really curious about is the fact that of course, stuttering is very psychologically draining, what would drive a person to become so covert or is it just developed naturally? Could it possibly be developed after previous treatments of trying to use cancellations, pull outs and the preparatory step methods? What I mean by this is, after a patient has attempted all these methods and ended therapy, is it possible that trying so hard to "stutter in an easier way" developed an almost restricted feeling rather then speaking easier which in turn would develop covert studying?
