Laura, Inhalations are common and difficult to get rid of. It seems that respiratory symptoms that are learned through escape appear to condition quickly and become long standing habits. I might start with encouraging the client to stutter directly on the sound that the word starts with. Getting on the sound usually triggers normal speech (on exhalation). As long as the client is not trying to do an easy onset (full breath) or is not responding to old advice (take a deep breath), making noise on the actual initial sound would in the end, reinforce a behavior incompatible with inhalation.
