Judy, As always the on line conference made me consider a number of things that I had not thought before. One of the conclusions I came up with this time that same as Psycho-Oncology, my other specialty besides Stuttering, the best approach for those clients who do not show progress is the team approach. Neither doctors nor psychologists involved in dealing with cancer can do the treatment of the all clients by themselves (see "Onco-Psychology" by Holland et al) so in stuttering therapy the SLP's may well need the assistance of psychologists, mental health counselors, or behavior health therapists (see article by Michael Sugarman and my comment) who practice CBT, DBT, ACT, REBT, etc. When a client is not making any progress the SLP's best call on the experts in the field of motivation (i.e. those who know Motivational Interviewing) and experts who are equipped to do the heavy lifting in handling self-defeating emotions.
