Thanks for your great paper ,its help me a lot to read papers like yours. 4 years ago I decided to stop avoiding stuttering,and to say the fear words and sounds that i couldn't say,it was very hard to stutter with people especially ecause my stuttering was very hard with very hard tensing silent blocks. I wanted to ask,you mentioned also stuttering Modification ,my impression was that Van Riper that founded it, didnt meant to work on his "modification tools" before the stutterer would reduce his fear to stutter,so the SLP'S today that jump to the Modification tools are really not like Van riper meant when he founded this method? And another question,today the most SLP's i know separate the tools part from the mental part,so you can work on the physical side with fluency Shaping tools or you can work on the mental side by talking about stuttering and CBT ,I from the other hand believe that the two parts are linkage together,for example "hold on the stuttering moment " can be a physical tool and it is also a very desensitize ,what do you think?
