I think that one of the best ways to help "reluctant clients" is to lead by example. First, the clinician can complete the challenge in a somewhat exaggerated manner to allow the client to just observe. Then having a former client or other PWS act as a mentor in that role can be especially powerful. It is important that people be led though desensitization exercises in a hierarchy and with full support. I find it especially helpful to provide praise (and even prizes!)when clients complete one of these exercises and have it NOT go so smoothly!
