Therapy targets may differ from case to case. In cases of neurogenic stuttering adults with accompanying aphasia and/or apraxia of speech I usually start treating those aspects of speech first, which have the most severe impact on the client`s communicative capability. If it is the word finding problem which impacts the individual the most, I would start treating that. If it is the fluency disorder, the clonic-tonic stuttering symptoms, I would start from there. However, in those cases where the individual has developed anxiety, avoidance behavior and so forth, the target might focus on the personal factors, strengthening self-confidence, reducing negative impact of stuttering on everyday life, and supporting activities and participation in communication situations.
