Hi Amberly. 1) I do not know of any case history form that includes questions about sleep patterns. But I make a lot of questions about sleep habits for my clients, such as: time to go to bed, time to wake up, if sleep schedules are regular from weekdays to weekends, if they feel tired along the day, if they take a long time to fall asleep, if it is difficult to wake up in the morning, if they need to take a nap in the afternoon, if they have some problem to breath, and so on. According to the answers, I counsel the client. 2) Sometimes it is necessary to refer the client to a neurologist or to a otorhinolaryngologist. The physician usually asks a polysomnography exam, which can be a valuable tool (specially in cases of central or obstructive apnea). Sandra
