You raise interesting points, Liv. Cultural differences probably do play a role. But there is also the rich history of stuttering research and treatment in the U.S. (and other countries, too, of course - but I'm especially referring to your comments about the U.S.) and the years of controversy that have accompanied that research.
As I alluded to in my ISAD essay, to most people who stutter therapies that address more than surface fluency make a lot of sense. To many (not all) of those who have not experienced stuttering, these types of treatment don't make sense. I think that tells us a lot.
