You can tell that this is Mike's story, not mine. I have asked him several times about your question (i.e., his teacher causing the stuttering). Each time he insists that his teacher was the cause. At this point in his therapy, I frankly don't dwell on the cause, as much as finding the cure. In addition, let me share a few things that Mike has said to me. I asked him once what a parent should do if their child had a "mean" teacher. His response was definite and swift. He said "remove them immediately and get them in another classroom." I was surprised, but it was his response and feels very strongly about it. Mike also told me that he feels he may not be stuttering today if it wasn't for that third grade teacher. I wonder if those opinions will change as he progresses through his therapy (which did include an educational component). Only time will tell, but you can guarantee that I will ask him the "causality" question again.
