You are right. You do need to be able to think metaphorically and engage in that process. Some clients do it more successfully that others, like all of us, I guess. I had one client who had a whole series of characters involved in the construct of communication. It was like a Lord of the Rings story! i really wanted her to write it down but she wanted to keep it in her head. That was a story that evolved and developed. I think you are right - it would be great if clients would do a sort of art/metaphor diary or record of the way the process changes for them and maybe the therapist could do one alongside them.
