Treating stuttering with cognitive delays

Children that have cognitive delays, such as Down's Syndrome, often are at greater risk for disfluencies. However, we also know that these children will have speech and language delays as well. For children that have cognitive delays, what do you feel would be more beneficial to work on first and foremost? I was also wondering which treatment approach, you believe would be more suitable for these children?
