I think it would have been helpful if SLPs hadn't been so focused on fluency. In my opinion, "fluency" is an unrealistic goal. But in striving to achieve fluency, a few things happened: a) Stuttering (ANY stuttering) became viewed as a terrible thing...or worse, a setback or relapse. b) Talking about stuttering (my feelings, etc.) became a taboo subject. Looking back, it's easy to see the steps that contributed to me becoming a covert PWS.
