This is an interesting question, and I hope that one of the SLP's here also replies to this because they'll be able to give a better answer than I will. For me, it was a matter of focus. It seems that clutterers are really not focused at all on their speech, and with me it was something that I just never thought about. The other thing is that cluttering disfluencies are all "normal" disfluencies, meaning that they are the same disfluencies that a very fluent speaker would have. This means that it's a lot harder for a clutterer to feel like their speech is different--it is different but mostly in quantity of disfluencies, and like Dr. Bakker pointed out, rate and rhythm. Also, cluttering isn't a household word like stuttering or lisp, and so people are much less likely to point out to a clutterer that the clutterer has a speech problem.
