As you say, every child is unique. My own experience is that children in this age want to stop stuttering and sound like other kids. The focus of my treatment would be on increasing stutter free speech. When kids express other concerns, e.g., anxiety about speaking in front of a group, etc. we try to help them to be as fluent as possible first. Those children with profound or persistent fears or anxiety about speaking (which may spill over to other situations) are often helped by the psychologist on our team.
