WOW!! After reading this and looking over the form, I couldn't believe I had never seen anything like it before. It is such a good idea for all students. I love how you included the factors of whether the students had witnessed anyone else being teased or done any teasing themselves. This is such a good tool and I hope to use some form of it with all my future speech students. Thanks.
