I sometimes give copies to parents when children come in infrequently or if there is to be a long break in therapy. The kids are usually willing to check in with mom (usually mom) if the routine has been established with me. Moms are often surprised to learn about teasing and want the form so they can keep up with what is happening and seek help from teacher, or whoever, when needed.
