Hello Gunnars, Once again you raise good questions for which I have no ready answers. I think, however, that just as you suggest, there is not necessarily one treatment that will work for everyone, there is likely not a singular explanation for why treatment fails and why people become discouraged about continuing the search. Again, I think you are carving out some interesting areas for possible research, but for you to do, not me. Regards, Bill
