I think it is very hard to use the 'phone when you have a successful form of commuication via email, but we all need to use the telephone on some occasions and I guess 'avoiding' it may only make things worse in the end

I suggest you try practising with an unconnected telephone. Take deep breaths and speak slowly and practise answering the phone with your name, and do this about 20 times again and again. Then practise making a phone call and using some words that you would use to introduce yourself and do this at least 20 times When you feel comfortable with that, move on and try some calls to people you know and try and build up your skill and confidence over time.
