With a client as young as your is, you may need to encourage him to go outside the clinic walls. Not really because he is afraid of stuttering, but at that age, many children tend to be shy about talking to people.

Let him get comfortable in the clinical setting with you, and then with the other clinic staff before you start easing him into the "real world". This way he will feel more "in control" of his situations, and won't be as apt to clam up and not say anything.
