That's a very good question. It would require some probing on the part of the SLP to determine the reason for not signing.Certain forms, I understand is not unusual for parents from different cultural groups not to sign forms (i.e. video or audio recording, observation or release of information). However, contractual and/or emergency forms, I would not consider to be problematic. If they are, I would consider my options for providing services to the client. The setting for services may have a role to play whether certain forms are signed or not. Finally, if its imperative for forms to be signed and parents refuse, but have a good understanding as to the importance for their signature, I would suggest refering them elsewhere.
