I am a pediatrician and have only a basic knowledge of dysfluencies. However, my son who is 10 has for at least six years shown exactly the word-final dysfluency described for your 12 y.o. boy, that is the trolley-olley type, and uses these also to bridge over phrases as you described. He also uses frequent restarts of sentences and some revisions, as well as pauses for breath at inappropriate places, and often seems rushed in speech. He does not, however, stutter. I have just also discovered the term cluttering, and wonder if all of these patterns including the very remarkable word-final dysfluencis, fit into the description of "cluttering"
