Hello M.: I agree that email therapy probably would have worked out more beneficially. As human beings, we all feel ill-at-ease if we advertise our differences, such as visibly going into a speech therapy classroom, and this is nothing to be ashamed of. I feel that this tends to reinforce the problem, and as Dr. Johnson pointed out in another response, everybody with a stuttering problem does have periods of fluency and "much emphasis must be put on this". My suggestion for you is to, if possible, begin E-mail Rx therapy with this boy as if you are starting with a "clean slate". In order for this really to become effective, the boy must understand that this is indeed starting over, learning fluency in a more comfortable manner, and is not just a different way of going through the same nervousness and anxiety present at the school sessions.
