Below are the questions I asked in the survey about attitudes of stuttering and people who stutter.

Unfortunately, some of the formatting and fonts have been lost in posting the questions.

1. How long have you known this individual (check ONE)?

Questions 3 and 4 below ask about the frequency of stuttering. Frequency is the number of times stuttering (repeated sounds or words, blockages in speech, etc.) occur, not their severity.
