1. Go to the registration website at https://secure.mnsu.edu/eventsconferences/?department=QualityMatters
2. Scroll down to the desired session date and click on the session link (e.g., “APP November 6 - November 20, 2012”)
3. Fill in the required contact information and Continue
4. Under “Institution Representing” select your institution from the dropdown menu.
5. Identify your role, dept and/or program name.
6. Verify your information and Continue
7. You should see the cost listed as $55.00 and can continue there also.
8. Select the payment option and enter the appropriate information.
· Faculty/staff from institutions other than Minnesota State University, Mankato may use a personal or other credit card or may provide a purchase order number and contact information for the person approving or associated with the purchase order.
· Faculty/staff from Minnesota State University, Mankato may use a personal credit card (i.e., one in which charges are made to and paid by you personally) or may use a cost center number. The administrative assistant for your college or department should be able to help you if you don’t know how to obtain a cost center number. You will also need to provide contact information for the person in your college, department or program who has approved your use of the cost center number. You may NOT use a department credit card or any university purchasing card (even if it has your own name on it.)
9. Submit your registration.
