Workers' Compensation Procedures

Need Medical Attention Now!

Step 1.  Go to:

Step 2.

Tell the doctor/nurse that your illness/injury may be work related.

Step 3.

Immediately report your injury to the workers’ compensation coordinator (507) 389-2016 and online at: www.mnsu.edu/hr/workcomp/

Hours: Monday through Friday 8am-8pm, Saturday 8am-5pm, & Sunday 11am-5pm

Reporting an Injury or Illness

• Immediately report the injury/illness to your supervisor.
  
• Within 24 hours of an injury/illness:
  • The Supervisor/Manager must complete the Injury/Illness/Incident Data Form (IDF) (a.k.a First Report of Injury Form) and Agency Claims Investigation Form
  • The Employee must complete the Employee Statement Regarding Injury/Illness
  • All completed forms are to be sent to Human Resources via scanning and emailing to diane.roggow@mnsu.edu, or hand delivered to WA 336, or faxed to 507-389-2960.

Life Threatening Illness or Injury

• If an injury is a potential life-threatening emergency, please call (507) 389-2111 or 911.

• If you are unable to drive, security will be notified, respond, and can direct the ambulance to you. 
  NOTE:  If you do not wish to be transported by an ambulance, it is not an option to have a co-worker or supervisor transport you.  You may call a cab or have a family member/friend transport you to the hospital.

• Tell the doctor/nurse that your illness/injury may be work related.

• Immediately report your injury to the workers’ compensation coordinator in Human Resources.  You may report your injury by going to this URL on our HR web page: www.mnsu.edu/hr/workcomp/workcomp.php or calling (507) 389-2016.

Non-Life Threatening Illness or Injury

• Immediately call/contact your supervisor.

• Your supervisor, security and you can determine whether or not you may need to seek medical attention.

Don’t need medical attention at this time...

• Immediately report your injury to the workers’ compensation coordinator in Human Resources on-line at www.mnsu.edu/hr/workcomp/workcomp.php or calling by (507) 389-2016.
  

• You and your supervisor should touch base within a couple of days from the date of your injury or illness.  If agreed upon, please be sure that your supervisor has your cell and/or home phone number so that s/he can touch base with you.  Please follow up on a daily basis until you are okay.

• If your injury/illness has not improved within three (3) days, please notify the workers’ compensation coordinator in Human Resources at (507) 389-2016 and visit the:

• If you decide you need medical attention after clinic hours, please go to either of the Mankato Clinic Urgent Care locations listed below:

• Be sure to notify the workers’ compensation coordinator in Human Resources at (507) 389-2016 the next business day or as soon a possible following your visit.

In Cases of Severe or Questionable Losses

• If the loss is severe or has very questionable circumstances, the Supervisor/Manager contacts the Safety Director at (507) 389-5568, for investigation assistance.
  

• If the Supervisor/Manager has any questions about the compensability or legitimacy of the claim s/he should contact Human Resources at (507) 389-2016.
