Claim Submissions

There are several ways to submit reimbursement claims:

Submit online. (Not Required)
To submit claims online and to track information you can create an account at the following link: Set Up Your Account!
Use Benny™ Card to pay at point of service.
Submit paper reimbursement claim forms to Eide Bailly.
Get Form! (2009)

All claims submitted online or on paper must include documentation such as itemized receipts or billing statements. Whatever method you use, be sure to keep copies of all receipts, billing statements, or other documentation in case you need to provide proof of an eligible expense at a later date.

NOTE:
The Employer (State of Minnesota) may amend the HRA Plan at any time and in any manner
that it deems reasonable.
