Reporting an Injury or Illness


Life Threatening Illness or Injury

  • If an injury is a potential life-threatening emergency, please call (507) 389-2111 or 911.

  • If you are unable to drive, security will be notified, respond, and can direct the ambulance to you. 
    NOTE:  If you do not wish to be transported by an ambulance, it is not an option to have a co-worker or supervisor transport you.  You may call a cab or have a family member/friend transport you to the hospital.

Non-Life Threatening Illness or Injury

  • Immediately call/contact your supervisor.

  • Your supervisor, security and you can determine whether or not you may need to seek medical attention.

Don’t need medical attention at this time...

  • Immediately report your injury to the workers’ compensation coordinator in Human Resources.  You may report your injury by going to this URL on our HR web page: www.mnsu.edu/humanres/workcomp/workcomp.php or calling (507) 389-2016.

  • You and your supervisor should touch base within a couple of days from the date of your injury or illness.  If agreed upon, please be sure that your supervisor has your cell and/or home phone number so that s/he can touch base with you.  Please follow up on a daily basis until you are okay.

  • If at that time your injury/illness has not improved within three (3) days, please notify the workers’ compensation coordinator in Human Resources at (507) 389-2016 and visit the:

  • Be sure to notify the workers’ compensation coordinator in Human Resources at (507) 389-2016 the next business day or as soon a possible following your visit.

In Cases of Severe or Questionable Losses

  • If the loss is severe or has very questionable circumstances, the Supervisor/Manager contacts the Safety Director at (507) 389-5568, for investigation assistance.

  • If the Supervisor/Manager has any questions about the compensability or legitimacy of the claim s/he should contact Human Resources at (507) 389-2016.
