Information and Procedures for MSU Faculty and Staff

This memo is to advise you of the test scanning services offered to all faculty by the ITS/Data Entry/Test Scanning staff. It will briefly outline procedures, various types of analysis, and turn around time.

  • TEST FORMS - Only use the General Purpose/NCS/Answer Sheet/ form no. 4521 - this form can be ordered from Office Supplies - MSU catalog number is 108 (NCS # 4521)

When a faculty member brings in a test to scan, the tests and key must be on the "blue" Scantron sheet (no. 4521). The blue Scantron sheet is the only Scantron sheet that our office processes. On the "key" the instructor will have the correct answers to the questions. In the upper left hand box "Name" the faculty member should leave the whole box blank.

  • JOB REQUEST FORM
    The faculty member must fill out a "Job Request". This form must include the name of the professor submitting the request and a phone number in case there are questions and to call when the test/analysis has been completed.
  • EXAMPLES OF REPORTS

  • TURNAROUND TIME
    Normal turn around time is 24 hours and this is especially true during peak demand times. During slow times when equipment is up and running and full staff is available the shortest turn around time possible is four (4) hours.
