Learning Community Enrollment Form

STUDENTS: Please read and type in your initials next to each of the following statements. Your initials indicate that you have read, understand, and agree to the conditions stated. Entering your name below is the equivalent of your signature.

  I understand that I will be registered by the Office of New Student and Family Programs and the Office of the Registrar for the courses that are a part of the learning community that I have selected (see "Types" page for details).

  I understand that if I have college-level credit (PSEO, AP/IB/CLEP, College in the Schools, etc) for any of the courses in the learning community that I have selected, I am still eligible to enroll in the program. I understand I am responsible for notifying the Office of New Student and Family Programs so I will not be registered for those courses.

  A specific number of seats are held in designated courses for learning community students. I understand that if I choose to drop any of the courses that I was registered for as a part of my learning community prior to August 31st, 2012 I will be required to drop all of the remaining learning community courses.

  To enroll in a residential learning community I must have a Residential Life housing contract for the 2012-2013 Academic Year. I know that I will receive a room assignment from the Office of Residential Life in the residential area reserved for the learning community I have selected (see "Types" page for details). I have reviewed cost information for the area in which I am requesting to live.

  To enroll in a learning community, I must sign and submit an Excellence in Citizenship Agreement to the Office of New Student and Family Programs (click "NEXT" below).
