**An event using more than just a podium with a microphone will require a sound technician (there is a fee for this service). If client does not provide times for the technician, then the technician will be scheduled for the duration of the event. All events with rehearsals will require a technician for the rehearsal as well as the actual performance.

**Special lighting requirements will require a light technician (there is a fee for this service). This includes spot lights, etc. during a performance or rehearsal.

**Note: University departments will need to reserve the media cart themselves using their MavCard unless the cart is used on the weekend or after 4pm during the week. To reserve a cart, call 507-389-5943 or reserve online. Student groups reserve through your advisor. Outside groups-the University Scheduling and Conference Services office will reserve for you.

By submitting this form, I will be the event coordinator and solely responsible for making any changes to the reservation for this event. In addition, I will be responsible for any incurred costs due to changes less than one (1) week prior to the event. I also understand that any late requests and/or changes may not be possible and it is at the discretion of the University Scheduling & Conference Service office and the CSU Operations department to grant or deny these requests.
