If your insurance is not listed above, we will provide insurance-ready receipts upon request. In this case, students are responsible for submission of their own insurance claims.
We recommend you check with your insurance company prior to your appointment in case a referral is required and to verify coverage. Submission of claims, does not automatically guarantee coverage. For questions about insurance coverage please contact your insurance customer service.
The Pharmacy can bill to most insurance companies if you have your current pharmacy card with you.
Students have the option to post charges from the clinic, lab, and pharmacy to their student account or pay the same day. Information about purchasing student insurance is as follows:
The Minnesota State Colleges and Universities System offers an Illness and Injury policy through UnitedHealthCare Student Resources. This is for students who are taking 6 or more Undergraduate credits or 3 or more Graduate credits.

The health insurance annual premium for the 2012-2013 policy year is $2,044. You will be able to purchase the insurance for 3 months, 6 mos, 9 mos or annually. The maximum coverage is $100,000 per illness or injury (including pharmacy). Some preventative coverage has been added. Please view the benefits at www.uhcsr.com. Go to "Find my school's plan".

Dental Insurance is also available. The cost is $271.92 per year. You can only purchase the dental insurance during the open enrollment period. The plan coverage dates and open enrollment dates are as follows:

Health Insurance for International Students is required by Minnesota state law. This insurance coverage is provided by UnitedHealthcare Student Resources and can be purchased through the Cashier's office. Returning students should purchase their coverage by the third week in July for the following year . For more information about health insurance for International students, contact Kathy Grant, Student Insurance Advocate, at Student Health Services 389-1432 or the International Student Office. NURSELINE: Students that have purchased the Student Insurance have access to nurse advice and health information 24 hours a day, 7 days a week by dialing the number listed on the permanent ID card. Collegiate Assistance Program is staffed by Registered Nurses who can help students determine if they need to seek medical care, understand their medications or medical procedures, or learn ways to stay healthy.
