Eligibility

Currently enrolled full-time and part-time Minnesota State University, Mankato students who are paying Student Activity Fees are eligible to use Student Health Services. Only students who pay this fee are eligible. Eligibility does not extend to spouses, life-partners, family members, children, etc., unless they are also enrolled students.

During summer session, students who were enrolled the previous spring semester are eligible (even if they are not taking summer classes) as long as they pay for their services at the time they are provided, and pay a $30 user fee.

Health insurance is not required to use Student Health Services. However, students are encouraged to carry health insurance of some type.

Confidentiality

All medical records are confidential and will be transferred to another facility only by the student's request and signed release. We respect students' right to privacy and will not give any information to anyone, including parents, roommates, friends, instructors, or administrators. While this may be frustrating to parents who "just need some information about my daughter/son for insurance reasons," we will not release information without signed consent.

Patient Rights & Responsibilities

The Minnesota State University, Mankato Student Health Services recognizes its responsibility to respect and protect the rights of all patients who seek treatment and information. Therefore, patients have the right to:

  • dignity. To be treated with respect and consideration.
  • be informed. To expect adequate explanation and instructions regarding your medical condition and care, procedures, diet and exercise, medications, laboratory test and follow up care.
  • ask questions. Your health care provider will be happy to address your questions and concerns.
  • confidentially and privacy. No information from your medical records will be released without your permission. your privacy will be protected.
  • refuse treatment. You may choose not to be treated by us or to only have a portion of your treatment provided by us.
  • choose your medical provider. You may request a provider at any time and may opt to change providers based on your needs or preferences.
  • voice concerns. You may share concerns with your provider or other staff, anonymously through our opinion boxes, or with the Director of the Student Health Services. If you feel like your concerns are not adequately address, you have the right to meet with the Vice President of Student Affairs to voice any dissatisfaction.

In addition to your rights as a patient it is your responsibility to

  • be honest. Give us complete and accurate information about your past and present health, medications, allergies or treatments.
  • help us improve. Tell us if you do not understand explanations and instructions.
  • comply. Success in treatment is often contingent on your willingness to comply and/or to let us know when you cannot comply.
  • be punctual. We can best serve you if you arrive on time for appointments and call us to cancel when you cannot make an appointment. This will allows us to provide services more efficiently and will help to decrease your waiting time.

If you have a concern/complaint, please offer your suggestions. The Student Health Staff encourage you, the patient, to be an informed and active participant in your own health care.

Please print and complete the above form and turn in or fax to Student Health Services to get a copy of your medical records released.
