Janet, I don't think we lack good definitions. There are several that are good. We just don't have agreement or the needed experimental support for any one. This situation is essentially the same in stuttering although the history of developing definitions is so much longer, and the behaviors to be included are more specific and identifiable. Precisely your objection is the reason why the cluttering severity instrument is based on perceptual determination of the presence of cluttering, and perceptual ratings of behavioral features. No child would be left untreated using this tool (which by the way is not diagnostic, but assessment oriented) while there are ways to quantify and be accountable for treatment success. I hope I didn't misunderstood your question, it was frankly not very clearly formulated.
